Termination Form

Submit this form for all student or supervisor initiated work-study employment termination proceedings.

Submission is NOT necessary if:

  • the student has been declared not eligible for work-study or has exhausted their allotment, or if
  • the student is or will be graduating during or at the end of the academic year.

Upon submission of the form, please be sure to terminate the student's employment in the myHR Temp Panel and assign a stop funding date on your chartstring in the Temp Funding panel. Contact askHR for assistance.
