Student-Employee Evaluation

We encourage employers to conduct student-employee performance evaluations at least annually or upon termination of employment. In most cases, it is recommended that the student and supervisor have a one-on-one meeting and review this form together.

Upon submission, a copy of this form will be emailed to the supervisor and student. 

Submission of this form is for Federal Work-Study Office record keeping processes ONLY and does not update the student's Human Resources employment record.  Employment termination or salary updates may be processed on the student's myHR Temp Panel, or contact the Kronos help desk for assistance.
